This provides us with pertinent demographic, medical and insurance information. All documents are kept compliant to the HIPPA regulations and will not be released without your permission. We accept most insurance plans including Medicare and Workers Compensation. If you do not have insurance, don't worry, we will work with you to get you the care you need at a manageable price. Payment plans can be worked out if needed.
